What is Health Home Services?

Health Home is a state program that serves Medicaid and Medicaid-Medicare Patients. We work with care coordinators to provide medical, behavioral health, and long-term services to clients.

How Does It Work?

Evaulation:

Conduct an evaulation to identify your health and social need.

Compressive Care Management Plan:

Co-develop a care management plan that addresses your health needs

Build Your Health Care Team:

Identify your health home team

Referrals to Community and Social Services:

Utilize our network and connect you to additional services

Comprehensive Transition Services:

Co- develop a plan to ensure a smooth and warm transition to primary care services
